Staff Social Worker

Position Overview

Working with out-patients and their families, the social worker provides direct social work service. The social worker functions as a member of the interdisciplinary team and provides psycho-social assessment and a range of interventions to assist patients and their families in dealing with personal, family or social difficulties that impact upon their level of wellness. The incumbent will act as an active and pro‑active member of the health care team in the Pulmonary Rehabilitation Program, providing both urgent service as well as short‑term follow‑up with identified patients and their families. The social worker is also a clinical resource to the Pulmonary Rehabilitation Program team members, the Misericordia Health Centre as a whole and to other social work professionals within the facility based on the training, experience and abilities demonstrated by the incumbent.
