Employment Type: Part time Shift: Description: The Medical Social Worker is responsible for the provision of consultation and service to homebound individuals in the need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen.• Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.
• Initially performs a comprehensive assessment of the patient and care-giver needs, and formulates appropriate plan of care.
• Establishes specific goals and regularly evaluates progress toward these goals.
