Responsibilities
- Direct work with patients and families including intake interviews, conducting financial assessment and providing necessary assistance and assisting with referrals to facilitate discharge planning in selective areas of care planning.
- Support Medical Social Workers in case management through translation assistance and is providing patient escort and home visit support.
- Participate in work process improvements and quality improvement projects.
- Work with other departments within the hospital and with external organizations to facilitate patient care and experience.
- Provide support on the administrative and operational running of the department, including managing the counter and related duties.
