Don’t miss this opportunity to be part of Baycrest’s Ambulatory Services. In this role, you will work as a member of interprofessional primary care outreach teams and the Neighbourhood Care Team. You will provide health navigation, nursing clinics, and chronic disease management for senior clients living independently in the community and within Toronto Senior Housing Buildings. Your focus will be on chronic disease management, frailty, mental health, and primary care. You will provide health education, wellness checks, and support clients in achieving their health goals to improve quality of life.
