Job Opportunity: Case Manager RN

We are seeking a dedicated Case Manager RN to join our team. This role involves working five 8-hour shifts per week, with every other weekend required.

Certification Requirements:

• BLS Certification

Experience:

• 5 years of RN Case Management experience


• Inpatient Acute Case Management (Hospital experience required)

Job Duties:

As a Case Manager RN, you will be responsible for coordinating care and planning transitions throughout the acute care patient experience.
