Palliative Care and Grief Support Specialist

Deliver impactful spiritual care as a Grief Support Specialist. Provide compassionate support in grief and bereavement while collaborating within a hybrid care model.
Reporting to the Clinical Manager, you will promote dignity and quality of life for clients confronting loss. This role integrates spiritual and psychosocial approaches in an interdisciplinary framework. You will work with various belief systems to address emotional and relational dimensions of care.
Key Responsibilities:
• Deliver individualized grief and bereavement counselling
• Perform biopsychosocial-spiritual evaluations and assessments
• Address various types of grief, including traumatic and ambiguous
• Collaborate on coordinated care planning with healthcare teams
• Ensure compliance with ethical and professional standards
Requirements:
