As a Nurse Practitioner, you will focus on elderly patients within the Hospitalist program, addressing sub-acute conditions and chronic care needs. Autonomously direct clinical care while also supporting care transitions in this diverse, team-oriented environment. The position requires a flexible schedule that may include on-call shifts.
Key Responsibilities:
• Provide inpatient care for designated older adult patients
• Collaborate with an interdisciplinary medical team
• Lead care planning and transitions for patient management
• Consult with physicians and healthcare personnel as necessary
• Manage symptoms of dementia and other complexities
Requirements:
• Master’s degree in Nursing required
• Current NP certification with the BC College of Nurses
