About the Role
Make an impactful change as a Nurse Practitioner with the Rexdale Community Health Centre. This full-time role focuses on delivering patient-centered care to underserved communities in Scarborough.
Working within the Interprofessional Primary Care Team (IPCT), you will address unique health challenges faced by diverse populations. You will provide a range of health services from assessments to preventive care strategies, ensuring culturally competent care.
Key Responsibilities:
• Offer direct primary care to individuals and families
• Develop and evaluate health promotion and treatment plans
• Conduct and interpret screening and diagnostic tests
• Support breastfeeding and prenatal care initiatives
• Advise and educate on chronic disease management
Requirements:
• Bachelor of Nursing Degree with relevant certifications
• Registration with the College of Nurses of Ontario
• Background in community nursing preferred
