About the Role
Transform continuing care service delivery as a Continuing Care Specialist based in Saskatoon. Bring your nursing expertise to lead key improvement initiatives and foster a safe environment for residents.
This position requires a background in Long Term Care, Rehabilitation, or Geriatric Services, along with a nursing degree. You will work closely with an interdisciplinary team to ensure adherence to evidence-based practices. Your role will also include onsite education for nursing staff and the development of relevant policies and procedures.
Key Responsibilities:
• Lead initiatives for restraint management and patient safety
• Collaborate in implementing leading care practices
• Educate nursing teams to promote effective care delivery
• Assist in the formulation of CC policies and procedures
• Conduct audits and recommend corrective actions as needed
Requirements:
• 3–5 years experience in a related healthcare field
• Nursing degree or diploma required
