**Position Purpose:**
The Trinity Health PACE Chaplain serves as a clinical spiritual care provider who supports patients, participants, families, and staff by addressing spiritual distress, grief, crisis situations, and major life transitions. As an integrated member of the interdisciplinary healthcare team, the chaplain assesses spiritual needs, delivers individualized care aligned with diverse beliefs, documents interventions in the medical record, and contributes to holistic care planning.
