We are seeking a highly motivated, compassionate and dedicated Case Manager to join our team in providing holistic, person-centred care for residents living with dementia. Key Responsibilities- Coordinate and oversee individualized care plans for residents with dementia
- Work closely with a multidisciplinary team (nurses, therapists, doctors, social workers) to ensure quality care. Participate in team meetings
