Job Title: Care Manager – Social Work (MUST HAVE LMSW or LCSW along with MSW)

Department: Care Delivery

Reports To: AVP of Care Delivery Operations

Job Purpose

Responsible for overall care management and quality of care for participants. Reviews assessments, coordinates holistic care plans across all domains, and ensures culturally sensitive, person-centered care.

Key Responsibilities

Participate in interdisciplinary care planning meetings and represent social work discipline

Review documentation for quality; address deficiencies with staff and follow disciplinary policy

Monitor, coach, and conduct training sessions for field staff

Coordinate care plans with interdisciplinary team (IDT) to meet member needs and goals
