About the Role
Job Summary
This position serves as the Targeted Case Management (TCM/CCM) and Community Support Program (CSP) APNP and is responsible for psychiatric care/medication prescription of TCM/CCM and CSP participants while collaborating with the program Psychiatrist.
Essential Duties and Responsibilities
Provide ongoing psychiatric evaluations of CSP participants after initial evaluation by the program Psychiatrist
Complete initial intake for TCM/CCM participants and then provide ongoing psychiatric evaluation
Prescribe medications and provide ongoing assertive medication management
Pragmatically evaluates options and decides upon treatment course that will optimize safety and community success within situational limits
Attend and participate in routine staffings with the Psychiatrist, nurses, supervisors, case managers, and other members of the CSP and TCM/CCMtreatment team while incorporating staff input regarding observations of participant ...
