Position Description: Reporting to the Program Supervisor, as a registered health professional (registered social worker, recreation therapist, practical nurse) you will be responsible to provide tenants with short-term case management supports that help them navigate community and social services, connect them to appropriate community referrals, and build capacity to self-manage physical and mental health needs.
